Guest Player Form (LIJSL)
Please note that the  Guest Player Form must be approved by the LIJ Soccer Leeague
for all guest players that have passes issued by the Long Island Junior Soccer League.
Teams playing in other leagues must obtain a Gueust Player Form from the League that the team and or player is sanctioned by.
